- Copy or KITAS by Immigration office that issued Kitas

- Birth Certificate (in English / Indonesia)
- Formal Photo (with red backround)
- Address in Bali (must be same with kitas address)
Valid Domicile Letter From Banjar/Neighborhood head - Domicile Letter Approved by Urban Village & subdistrict (if Any)
- KTP (ID Card) & Kartu Keluarga (Family Certificate) Sponsor from same district
